This position is fully remote, allowing you to work from the comfort of your home while accessing a range of technological tools that facilitate patient communication, collaboration with multidisciplinary teams, and data management. You will have access to our electronic health record (EHR) system, which supports documentation and client management, along with other remote working technologies to enhance your effectiveness.

As a Remote Social Worker specializing in hospice care, you will engage in a variety of responsibilities aimed at supporting patients and their families. Your key tasks will include:

• Conducting thorough patient assessments via telehealth to evaluate emotional, social, and psychological needs.
• Developing and implementing tailored treatment plans that address the unique challenges faced by patients in hospice care.
• Coordinating with healthcare providers, including physicians, nurses, and other allied health professionals, to ensure holistic and interdisciplinary care approaches.
• Advocating for patients' rights, helping them access community resources, assistance programs, and support services.
• Maintaining accurate and timely documentation of client interactions and treatment plans in our digital health record system.
• Providing psychoeducation and emotional support to family members, equipping them with coping strategies to navigate the challenges of end-of-life care.
• Engaging in regular team meetings, contributing insights, and sharing best practices to enhance service delivery.

The primary focus of this role is to support individuals receiving hospice care, as well as their families. You will encounter diverse patient backgrounds and challenges, including managing terminal illnesses, addressing grief and loss, and navigating the emotional landscape of end-of-life decisions. Your expertise will play a crucial role in helping clients achieve peace and dignity during their final stages of life.

To qualify for this position, candidates must possess the following credentials and qualifications:

• A Master’s degree in Social Work (MSW) from an accredited institution.
• A valid and active Licensed Clinical Social Worker (LCSW) license in Oregon.
• A minimum of three years of relevant social work experience, particularly within healthcare or hospice settings.
• Proficient communication and interpersonal skills, with a strong ability to demonstrate empathy and compassion.
• Adeptness in using remote work technologies and electronic health records.

While not mandatory, candidates with additional experience in hospice care, bereavement counseling, or trauma-informed care will be given preference. Familiarity with the Oregon healthcare system and community resources available to patients and families is also advantageous.

We offer a competitive yearly salary ranging from $100,000 to $105,000, reflective of your skills and experience in the field. In addition, our benefits package includes:
