We are seeking an experienced and empathetic part-time registered nurse (RN) to join our team as a remote care manager. In this role, you will engage with patients virtually to assess, coordinate, and monitor their health care needs-playing a vital part in our mission to enhance quality of life through proactive care management.
Key Responsibilities:

  • Conduct remote patient assessments with empathy and professionalism.
  • Monitor patients' health status and escalate warning signs appropriately.
  • Apply medical terminology knowledge to assess and coordinate care.
  • Communicate clearly and compassionately with patients and families.
  • Document thoroughly and accurately within multiple computer systems.
  • Prioritize daily responsibilities while working independently.
  • Provide excellence in patient care and customer service.

Qualifications:
  • Current, active RN license in good standing
