Job description

Pharmacist – Medication Therapy Management (MTM)
Location: Remote (WFH) – Must reside in TX, IL, FL, or PA and hold an active pharmacist license in that same state.
Schedule: Monday–Friday, 8-hour shifts starting between 9:00 AM – 11:00 AM CT
Training: 8:00 AM – 4:30 PM CT (approx. 2 weeks)


About the Role

We are hiring MTM Pharmacists to deliver telephonic Medication Therapy Management services for a major healthcare client. In this role, you will conduct comprehensive medication reviews, identify medication-related issues, and collaborate with members, caregivers, providers, and internal teams to improve patient outcomes.

This is a fully remote opportunity—ideal for pharmacists experienced in MTM, telehealth, and call-center environments.


Key Responsibilities
  • Perform telephonic comprehensive medication reviews (CMRs) and medication reconciliation for high-risk members.

  • Identify and document medication-related problems and coordinate appropriate resolutions.

  • Create member-friendly written summaries and provider communication/fax materials.

  • Communicate with prescribers, nurses, and care teams to support optimized medication therapy.

  • Use nationally recognized guidelines to develop member-specific treatment plans.

  • Maintain accurate documentation and meet productivity/quality metrics.


Required Qualifications
  • Bachelor’s degree in Pharmacy or PharmD.

  • Active, unrestricted pharmacist license in TX, IL, FL, or PA.

  • Previous call center experience (inbound/outbound) – must be able to describe experience.

  • Ability to work from home effectively with strong focus and goal orientation.

  • Must have a quiet, distraction-free workspace that can be hard-wired to the internet (Ethernet required).


Preferred Qualifications
  • Previous MTM experience, ideally using Outcomes MTM Connect.

  • Proficiency with Microsoft Office and basic computer systems.
