As part of our hospice care team, you will collaborate closely with physicians, nurses, and other healthcare professionals who share a commitment to delivering exceptional care to patients during their most vulnerable times.

• Conduct comprehensive psychosocial assessments of hospice patients, identifying their unique emotional, social, and spiritual needs in order to inform individualized care plans.
• Provide face-to-face and virtual counseling services aimed at helping patients and their families cope with the mental and emotional challenges associated with terminal illness.
• Collaborate with interdisciplinary teams to devise and implement personalized care strategies that encompass behavioral modification, stress management, and end-of-life transitions.
• Facilitate support groups for patients and caregivers that foster open communication and shared experiences, focusing on mental health and emotional well-being.
• Document patient interactions, including assessments and treatment plans, accurately and promptly in the electronic health record (EHR) system, ensuring continuity of care and compliance with regulations.
• Engage in regular outreach to patients and families to promote ongoing support, enhancing engagement with mental health resources available through the hospice organization.

In this role, you will primarily serve patients within the hospice setting, providing support to those facing terminal illnesses and their families. You will work with a diverse patient population, understanding their cultural backgrounds, spiritual beliefs, and specific psychological needs as they navigate the complexities of end-of-life care.

• Education: A Master's degree in Social Work from an accredited program.
• Licensure: An active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• Experience: A minimum of 3 years of experience in a mental health or outpatient setting, ideally with exposure to hospice or palliative care.
• Skills: Strong understanding of evidence-based therapeutic modalities along with excellent interpersonal, communication, and clinical assessment skills.

• Previous experience in hospice care, providing emotional support for individuals and families dealing with illness.
• Familiarity with the use of electronic health record (EHR) systems for tracking patient care and outcomes.
• Experience in developing and facilitating therapeutic group processes.
