This position offers the flexibility of a remote work setting, enabling counselors to manage their time effectively while interacting with clients. Key Responsibilities
  • Conduct comprehensive evaluations to identify and address the mental health needs of clients.
  • Facilitate communication with supervisors, direct care staff, family members, and guardians to provide updates on client progress and treatment plans.
  • Develop and share a structured plan with supervisors, detailing scheduled appointments, home visits, and monitoring sessions to track therapeutic milestones.
  • Organize home visits for in-home assessments, chart reviews, and interviews to ensure comprehensive understanding of client circumstances.
  • Participate actively in Interdisciplinary Team Meetings to discuss client treatment strategies and progress collaboratively.
  • Prepare and submit monthly and quarterly progress reports, highlighting client outcomes and areas for improvement.
  • Lead training sessions to aid staff and colleagues in executing program plans and documentation effectively.
  • Perform follow-up interviews and observations consistent with client care protocols to ascertain ongoing effectiveness of treatment.
  • Maintain detailed and organized client records to ensure accessibility of vital information for all stakeholders.
  • Act as a liaison between clients and the broader mental health community to provide resources and support that align with developmental goals.
  • Contribute to the creation and refinement of training materials aimed at improving program effectiveness and staff capability.
