Optimum Care Counseling is actively seeking a Licensed Master Social Worker to join our dedicated team in Tallahassee, Florida. This full-time position offers the opportunity to work remotely while providing critical support to our senior clients. This role will primarily operate in a remote setting, providing flexibility for the right candidate. You will work Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours depending on client needs. While the position allows for a part-time schedule if preferred, candidates must demonstrate flexibility to ensure ongoing support and care for our clientele. On-call duties may also be required to address urgent situations or provide crisis intervention.

• Conduct in-depth assessments of seniors’ psychosocial needs, taking into account their emotional, mental, and social conditions.
• Develop tailored care plans in collaboration with clients and their families, focusing on individual goals, cultural values, and family dynamics.
• Provide individual and group counseling, facilitating emotional support for clients undergoing life transitions or facing challenges.
• Act as an advocate for seniors' rights, ensuring access to essential community resources, services, and support systems.
• Coordinate with interdisciplinary teams of healthcare providers, including nurses and caregivers, to develop and implement comprehensive care strategies.
• Regularly visit clients to monitor their well-being, assess living situations, and address any emerging concerns or safety issues.
• Foster effective communication between family members and stakeholders involved in the client's care, mediating conflicts as needed.
• Provide education on community resources and support systems that can benefit clients and enhance their social engagement.
• Maintain precise documentation of assessments, care plans, client interactions, and any progress updates, ensuring compliance with industry regulations.
• Engage in continuous professional development to stay updated on best practices, policies, and ethical standards in the field of social work.

This role primarily focuses on seniors, addressing their unique psychosocial needs and helping improve their overall well-being through personalized care. Our community clinic understands that elderly clients face various challenges, such as health issues, isolation, and life transitions. You will work closely with these individuals and their families to create an impact in their daily lives.

• Master's degree in Social Work from an accredited institution.
• Current and valid licensure as a Master Social Worker (LMSW) in the state of Florida.
• A minimum of two years of professional experience in social work, ideally within a community setting or focused on geriatric care.
• Strong understanding of social work principles, aging-related issues, and knowledge of community resources.
• Excellent assessment, interviewing, and counseling skills tailored to seniors and their families.
• Capacity to empathize and establish rapport while effectively communicating with clients from diverse backgrounds.
• Strong problem-solving abilities and collaboration skills for working alongside interdisciplinary teams.
• Familiarity with electronic health record (EHR) systems and proficiency in relevant computer applications.

• Experience working in outpatient community health programs.
• Familiarity with case management strategies specific to elderly care.
• Knowledge of local social services organizations and resources available to seniors.
