Job Summary:
Provides care management support to panel of patients who require specialty services integrated and in support of their overall plan of care. Interacts with and supports the specialty care providers, primary care providers and the interdisciplinary care team across the continuum of care. Assesses plans, implements, documents, coordinates, monitors, evaluates, and updates the plan of care by collaborating with all members of the health care team to provide evidenced based care, leveraging nationally recognized guidelines as appropriate. Researches, evaluates and recommends resources to meet medical and non-medical needs of patients and families. Works to establish collaborative processes that promote quality and cost effective care that optimizes the physical and psychosocial health of patients across the health care continuum. Responsible for supporting decisions that impact health care outcomes, resource allocation and customer experience. Acts as a primary contact for the care team to assist in navigation and complexity management. Engages in quality improvement initiatives and program development. Could require work and travel to multiple locations in support of the patient and department.
Key Position Details:
  • Position will support cancers across the care continuum including but not limited to: head and neck, skin, and sarcoma. Diagnoses supported may evolve over time.

Job Description:
Principle Responsibilities
  • Care Coordination and Navigation
    • Provides care management and overall plan of care support to a panel of patients who require integrated oncology services.
    • Acts as a primary contact for the patient and the interdisciplinary health care team to assist in navigation through the complexities of the healthcare system.
    • Promotes patient engagement and shared decision-making to improve outcomes and satisfaction.
    • Researches, evaluates, and recommends resources to meet medical and non-medical needs of patients and families.
  • Assessment, Planning, Implementation, and Evaluation
    • Collaborates with the oncology care providers, primary care providers and the interdisciplinary care team across the continuum of care.
    • Implements, documents, monitors, and evaluates care plans in collaboration with the interdisciplinary team.
    • Provides evidence-based care aligned with Oncology national organizational standards of care.
    • Works to establish collaborative processes that promote quality and cost-effective care that optimize the physical and psychosocial health of patients across the continuum.
  • Team Participation and Quality Improvement
    • Engages in quality improvement initiatives and program development.
    • Advances the Allina Health Cancer Institute through the integration of evidence-based practice initiatives that align with standards set by national organizational guidelines for quality oncology care.
    • Actively engages in care team and departmental meetings to support interdisciplinary collaboration.
    • Contributes to problem-solving, idea generation, and continuous improvement efforts within the oncology care team.
  • Other duties as assigned.

Required Qualifications
  • Bachelor's degree in nursing
  • 2+ years inpatient or outpatient oncology nursing experience

Preferred Qualifications
  • 3+ years nursing experience in clinical specialty area (i.e., Oncology, Hematology, Care Coordination or Care Management)
  • Progressive experience in performance improvement and/or program development

Licenses/Certifications
  • Licensed Registered Nurse - MN Board of Nursing required upon hire
  • Licensed Registered Nurse - WI Dept of Safety & Professional Services required upon hire if providing care to patients over the phone, through e-visits, virtual visits, or medical messages and the patient is in Wisconsin at the time of the care (To Be Determined Based on Position)
  • Must meet at least ONE of the requirements below:
    • Current BLS certification from the American Heart Association
    • Current BLS certification from the American Red Cross
    • Allina in-house BLS training (within 30 days of hire)
  • Case Manager Certification or Specialty Certification preferred; For Cancer Institute: Oncology Certified Nurse (OCN) certification is preferred

Pay Range
Pay Range: $46.42 to $64.22 per hour
The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
