Vinculum Care, a locally owned home health agency serving the Port Huron area since 2019, is seeking a compassionate, dependable, and professional Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our growing team.

Position Overview

This position is responsible for providing skilled nursing services, conducting client assessments, developing and monitoring care plans, and ensuring clients receive quality care in accordance with physician orders and agency policies.

Both full-time and part-time opportunities are available.


Responsibilities

  • Perform nursing assessments and reassessments of clients.
  • Develop, implement, and monitor individualized care plans.
  • Administer medications and treatments as prescribed.
  • Coordinate care with physicians, caregivers, and other healthcare professionals.
  • Maintain accurate and timely clinical documentation.
  • Monitor client health status and report significant changes.
  • Provide education and support to clients and their families.
