Job description

Role: Patient Transfer Coordinator

Location: Remote Indiana

Duration: Long-term Contract

Duties & Responsibilities

·        Receive, review, and coordinate patient transfer, placement, and escalation requests received through the  MOCC. 

·        Assess patient needs, acuity, service requirements, and facility capabilities to identify appropriate  receiving organizations and levels of care. 

·        Facilitate communication between referring providers, receiving providers, healthcare facilities, and  other stakeholders to support timely clinical decision-making. 

·        Coordinate transfer logistics and resource requirements necessary to support safe and appropriate patient movement. 

·        Obtain, review, and communicate pertinent clinical information required to support placement and  transfer decisions. 

·        Monitor transfer and placement activities to ensure timely progression and resolution of requests. 

·        Maintain situational awareness of regional healthcare resources, facility capacity, specialty service  availability, and operational constraints. 

·        Escalate complex, high-acuity, or time-sensitive situations to clinical leadership and appropriate  stakeholders. 

·        Document transfer activities, communications, and outcomes within designated technology platforms. 

·        Collaborate with hospitals, healthcare providers, emergency departments, post-acute providers, and  community organizations to improve care coordination and access. 

·        Support regional surge, diversion, and emergency response activities as directed. 

·        Participate in quality improvement initiatives, performance monitoring, and operational reporting activities. 

·        Maintain compliance with all applicable policies, procedures, privacy requirements, contractual obligations, and regulatory standards. 

·        Provide exceptional customer service while supporting healthcare providers, facilities, and patients across the region.

Knowledge, Skills & Abilities

·        Knowledge of patient transfer, placement, and care coordination processes.

·        Ability to assess patient needs, prioritize requests, and make sound decisions.

·        Strong communication and collaboration skills with healthcare providers and facilities.

·        Critical thinking and problem-solving skills in fast-paced environments.

·        Proficiency with healthcare technology, documentation, and compliance requirements.

 

Minimum Qualifications

·        Three (3) years of clinical experience in an acute care hospital, emergency department, critical care, case  management, patient placement, transfer center, bed management, care coordination, or related  healthcare setting. 

·        Knowledge of clinical terminology, healthcare operations, and patient care delivery processes. 

·        Ability to assess and prioritize multiple requests in a fast-paced environment. 

·        Strong communication, critical thinking, and problem-solving skills. 

 

Preferred Qualifications

·        Current licensure as a Registered Nurse (RN). 

·        Five (5) or more years of healthcare experience. 

·        Experience in patient placement, transfer center operations, bed management, throughput management,  capacity management, or care coordination. 

·        Knowledge of regional healthcare resources, specialty services, and referral networks. 

·        Experience coordinating interfacility transfers, specialty referrals, or complex patient placements. 

·        Experience working with electronic health records, transfer management platforms, and healthcare  communication systems. 

·        Pediatric, critical care, emergency department, trauma, or specialty care experience preferred.

·        Experience supporting healthcare system operations, patient flow, and capacity management initiatives.
