Job Description:

The RN Triage and Message Management provides remote clinical support for outpatient practices by performing telephonic triage and managing outpatient messages in the electronic health record (EHR) and patient portal. This role uses evidence-based triage protocols and nursing judgment to assess symptoms, determine urgency, provide patient education and self-care guidance, coordinate timely follow-up, and escalate care when indicated. The RN supports safe, efficient, patient-centered care by ensuring accurate documentation, timely message resolution, and close collaboration with providers and the interdisciplinary care team. The RN also supports value-based care initiatives through proactive panel support, care coordination, and workflows that improve access, close care gaps, and promote quality outcomes and patient experience.

Position Details

  • Full Time, Monday - Friday 8:00 - 5:00

  • Inbasket Management - may evolve to add phone triage and patient outreach in future.

  • No weekends/holidays/evenings or graves.

  • No on-call requirements

  • Fully Remote

  • This position is a remote position; however the caregiver must reside in Colorado or Montana

  • Must live within 20-30 minutes from Intermountain Health care site. (Hospital or clinic).

Essential Functions

  • Perform remote clinical triage (phone, video, and/or asynchronous portal messaging) using focused assessment, red-flag identification, and organization-approved triage protocols to determine appropriate level and timeliness of care.

  • Manage and prioritize high-volume patient messages (EHR in-basket/patient portal), providing nursing guidance within scope and routing requests (e.g., symptoms, refills per protocol, results follow-up per workflow, documentation needs) to the appropriate team members.

  • Escalate urgent or high-risk concerns promptly per policy, including directing patients to emergency services (911/ED) when indicated and ensuring timely provider notification.

  • Coordinate care and support value-based care initiatives by facilitating timely appointments/referrals, supporting transitions of care, and helping close care gaps through outreach and follow-up for preventive and chronic care needs.

  • Provide clear patient education and self-care instructions using plain language (including return precautions and follow-up guidance) and confirm understanding when appropriate.

  • Document assessments, advice, and actions accurately and on time in the EHR; use telehealth/messaging tools effectively; follow standardized workflows to meet turnaround expectations and support quality, safety, and patient experience goals.

  • Data Analysis: Conducts root cause analysis of extended post-acute stays, inappropriate utilization, readmissions, and track key data elements or metrics. Identifies, analyzes, and monitors industry, regulatory, technology, and market-based trends that impact ambulatory and post-acute services.

Minimum Qualifications

  • Current Registered Nurse (RN) license in state of practice.

  • Bachelor of Science in Nursing (BSN) from an accredited institution (degree verification required). RNs hired or promoted into this role must obtain their BSN within four (4) years of hire or promotion date.

  • Demonstrated clinical nursing experience in chronic disease management, and familiarity with chronic disease terminology and processes.

  • Demonstrated understanding of disease management including treatment, length of stay, identifying barriers to delivery of care and any variation.

  • Basic computer skills and knowledge of Microsoft Office software.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) from an accredited institution.

  • Experience in ambulatory care or telephonic triage.

  • Intermediate computers skills and knowledge of Microsoft Office software

Physical Requirements

  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

  • May be expected to stand in a stationary position for an extended period of time.

  • For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
