Position Overview

Los Barrios Community Health Clinic is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our dynamic team in a full-time, remote school-based role. Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify emotional, social, and financial needs of students and their families.
  • Collaborate with medical and educational teams to develop and implement individualized treatment plans that address patient needs holistically.
  • Provide individual and group counseling sessions focused on a range of issues, including adjustment to school, grief, anxiety, and coping strategies related to mental health challenges.
  • Facilitate communication between families and school staff to ensure everyone is informed and prepared to support the student’s needs.
  • Coordinate with community organizations to arrange for post-discharge services, ensuring continuity of care and access to necessary resources.
  • Offer emotional support and educational resources about treatment options and community services available to families.
  • Maintain accurate documentation of patient interactions and ensure compliance with clinic policies as well as legal requirements.
  • Participate in interdisciplinary team meetings and provide insights on social work issues to improve patient care outcomes.
Patient Population / Client Focus

This role will focus primarily on school-age children and adolescents, alongside their families, addressing challenges such as behavioral issues, emotional disturbances, and situational crises. Your work will play a pivotal role in fostering a supportive environment that encourages academic success and personal growth, ensuring that students feel seen, supported, and empowered.

Qualifications
  • Master's degree in Social Work from an accredited program.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
Preferred Experience
  • A minimum of two years of relevant experience in a healthcare or educational setting is preferred.
  • Familiarity with school-based mental health services, community resources, and crisis intervention techniques.
  • Proficiency in medical terminology and understanding of mental health disorders.
