This position carries a demanding but rewarding caseload, with a potential yearly salary between USD 100,000 and 105,000.

Key Responsibilities
  • Collaborate with interdisciplinary team members to assess and anticipate the needs of clients and advance their care plans effectively.
  • Provide emotional support and education to patients and their families, facilitating a smooth transition plan that addresses their specific requirements.
  • Stay informed about government and private payer networks to navigate funding options and ensure appropriate transitions for our clients.
  • Coordinate care by connecting patients with community resources and healthcare services tailored to their needs.
  • Advocate for the rights of patients and their families while ensuring access to protective services where necessary.
  • Maintain ethical standards by demonstrating an understanding of patient rights and engaging in advocacy efforts on their behalf.
  • Responsible for reporting any signs of vulnerability or suspected abuse as per legal mandates.
  • Participate in initiatives aimed at reducing hospital readmissions by providing appropriate educational resources and counseling for at-risk patients.
  • Organize and facilitate transition conferences with patients and families to discuss care pathways and follow-ups.
  • Utilize advanced tools and technology to pinpoint patients at risk for readmission and develop intervention strategies accordingly.
  • Guarantee efficient clinical handoffs for high-risk patients, which often necessitate referrals to additional services as needed.
  • Document and maintain patient records in our electronic medical system, ensuring progress toward goals and adherence to social work plans.
  • Perform additional duties as assigned to support the overall functioning of the outpatient services.
Patient Population / Client Focus

Your primary focus will be on individuals and families affected by severe mental health challenges, including those grappling with chronic or acute illnesses. You will be tasked with providing tailored therapeutic interventions and family-centered healthcare strategies, ensuring that each client's diverse needs and backgrounds are respected and addressed.

Qualifications
  • A Masterโ€™s or Doctorate degree in Clinical Psychology, Counseling, or Clinical Social Work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.
  • Possession of an independent state license (e.g., LCSW, LMFT, LPC) to practice in the United States.
  • Demonstrated ability to efficiently manage a diverse caseload, collaborating with larger healthcare providers and support teams effectively.
  • Strong proficiency in electronic health records and communication software, ensuring streamlined documentation and information exchange.
