Position Overview

HEARD Emotional Wellness Solution, Inc. is seeking a dedicated and licensed Mental Health Therapist to join our team in a fully remote capacity. As an integral member of our outpatient office, you will provide comprehensive mental health support to individuals facing acute, chronic, or terminal illnesses. Key Responsibilities
  • Provide direct counseling and mental health support to patients, ensuring they receive tailored strategies to manage their emotional health.
  • Conduct thorough assessments and develop individualized treatment plans in collaboration with patients and their families.
  • Utilize electronic medical records to monitor patient progress, document therapeutic interventions, and communicate effectively with the interdisciplinary team.
  • Ensure mandated reporting of any suspected vulnerability or abuse in accordance with state laws and regulations.
  • Facilitate transition conferences and discharge planning with patients and families, promoting continuity of care and avoid unnecessary readmissions.
  • Identify patients at risk for readmission by leveraging available tools and technologies to implement proactive interventions.
  • Advocate for patient rights, ensuring they have access to necessary resources and protective services.
  • Collaborate with community resources and healthcare providers to coordinate comprehensive care, addressing both psychological and social needs.
  • Implement strategies to assist family caregivers and provide educational resources to empower them in their caregiving roles.
  • Engage in regular supervision and collaboration, sharing insights and best practices with a multidisciplinary team.
Patient Population / Client Focus

Your focus will be on a diverse clientele including individuals with mental health challenges related to acute, chronic, and terminal illnesses. By adopting an empathetic and personalized approach, you will address their unique needs and vulnerabilities, while also supporting their families to navigate the complexities of mental health care.

Qualifications
  • A Master’s or Doctorate degree in clinical psychology, counseling, or clinical social work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.
  • An independent license to practice in Oregon, such as LCSW, LMFT, or LPC.
  • A strong ability to manage a diverse caseload efficiently while maintaining high standards of care.
  • A passion for making a difference in the field of behavioral health, demonstrating commitment to patient care.
  • Proficient in using electronic medical records and other related systems to enhance workflow and patient documentation.
    • Preferred Experience
      • Experience working with outpatient mental health programs and a comprehensive understanding of the healthcare system.
      • Demonstrated ability to build rapport with clients and foster a supportive therapeutic environment.
      • Experience collaborating with various stakeholders, including community resources, healthcare professionals, and patient advocacy organizations.
