Key Responsibilities
  • Conduct patient assessments to understand their mental health needs and develop tailored treatment plans that incorporate therapeutic techniques and care strategies.
  • Provide compassionate counseling and support to both patients and their family caregivers, equipping them with strategies to manage emotional stress and mental health challenges.
  • Advocate for patient rights and actively participate in ethical decision-making processes to promote the best interests of patients and their families.
  • Ensure compliance with mandatory reporting laws, especially regarding cases of vulnerability or suspected abuse.
  • Participate in transition planning conferences, helping to guide families through changes in care and provide necessary resources.
  • Utilize cutting-edge electronic medical records (EMR) systems to document sessions, track progress, and communicate effectively with interdisciplinary team members.
  • Collaborate with healthcare providers to coordinate comprehensive care, minimizing hospital readmissions and promoting recovery.

  • Monitor and evaluate the efficacy of intervention plans, adapting them as necessary to align with the evolving health status of patients.
Patient Population / Client Focus

This role will focus on individuals grappling with a variety of mental health issues linked to serious health conditions. As a Licensed Mental Health Therapist, your work will involve tailored interventions aimed at enhancing their emotional resilience and overall quality of life. Qualifications
  • A Master's degree or doctorate in clinical psychology, counseling, or clinical social work from an accredited program is required (e.g., MA, MSW, MFT, PhD).
  • Must hold an independent license in the state of residence, such as LCSW, LMFT, or LPC.
  • Exceptional electronic mail communication skills to ensure timely and professional interactions with clients and team members.

Compensation and Benefits
