• Perform thorough assessments of patients and their families to identify emotional, social, and financial needs.
• Develop and implement tailored treatment plans using solution-focused therapies that cater to the unique circumstances of each client.
• Provide individual and family counseling, addressing issues surrounding grief, loss, anxiety, and depression related to terminal illness.
• Coordinate with a multidisciplinary treatment team to ensure comprehensive care that aligns with the patient’s goals and wishes.
• Maintain detailed and confidential records compliant with HIPAA regulations, ensuring quality documentation of care and interventions.
• Engage in ongoing education, staying abreast of current trends and methods in social work practice within the hospice setting.

In this role, you will primarily support patients who are receiving hospice care, often facing complex emotional challenges due to terminal illnesses. Your work will involve collaborating with patients and their families, offering guidance and resources as they navigate the emotional landscape of end-of-life care. You will be essential in fostering a supportive atmosphere that encourages open discussions about fears, wishes, and coping mechanisms.

• A Master’s degree in Social Work from an accredited institution.
• Active and unrestricted LCSW license in Colorado.
• Strong communication skills, with the ability to convey compassion and understanding in sensitive conversations.
• Adept at critical thinking and problem-solving, particularly in high-stakes situations.
• Experience or training in palliative or hospice social work preferred.

While the required qualifications include a Master’s degree and LCSW license, experience specifically in hospice or palliative care is highly valued. Familiarity with grief counseling techniques and emotional support frameworks is also advantageous.
