This position is primarily remote, allowing counselors to provide services from their own locations while staying connected with clients through teletherapy and digital platforms. As a Licensed Clinical Professional Counselor, you will:

• Assist primary care providers in recognizing, treating, and managing various mental health and psychosocial issues, enhancing overall patient care.
• Conduct thorough patient intakes that include diagnostic evaluations, ensuring that treatment recommendations align with established goals.
• Provide brief and focused mental health interventions tailored to the needs of clients, fostering their emotional resilience.
• Evaluate patient progress and collaborate with primary care providers to adjust treatment plans as necessary.
• Develop and implement relapse prevention plans, enabling clients to achieve and maintain stable functioning.
• Identify 'at-risk' patients and create proactive strategies to avert deterioration of their mental health conditions.
• Document patient progress meticulously in treatment charts, maintaining compliance with federal guidelines.
• Educate clients on self-management skills and health promotion strategies, empowering them in their treatment journeys.
• Consult with clinic management and team members on best practices in behavioral health services and program evaluation.
• Encourage patient adherence to medical treatment plans from primary care providers, assisting with coping strategies for medication side effects.
• Be committed to upholding the Code of Ethical Conduct for Clinical Professionals.

Your expertise will cater to a diverse patient demographic, including individuals and families facing various mental health challenges within the community. You will work closely with those requiring mental health support and connect them with effective resources, ensuring each patient receives personalized attention to meet their unique needs.

To qualify for this position, candidates must possess:

• A minimum of a Master’s Degree in Clinical Counseling.
• A current and valid Licensed Clinical Professional Counselor (LCPC) License.
• In-depth knowledge of mental health assessment, treatment planning, and various therapeutic modalities.
• Strong understanding of developmental psychopathology and behavioral therapy techniques.
• Ability to assess and evaluate patient needs effectively.
• Excellent documentation skills to maintain clinical records.
• Strong communication skills and the ability to establish collaborative relationships with patients, families, and health professionals.

While not required, candidates with the following attributes will have a competitive advantage:

• Fluency in both English and Spanish.
• Experience working within Federally Qualified Health Centers (FQHC) or similar environments.
• Knowledge of community resources and behavioral health services to enhance patient care.
