• Conduct thorough assessments to identify the specific mental health needs of each client, ensuring that personalized treatment plans are developed and adhered to.
• Maintain clear and effective communication with a variety of stakeholders, including clients, family members, supervisors, and external community resources regarding progress and treatment goals.
• Create and provide a structured schedule for appointments, home visits, and observations that allow for consistent monitoring of client progress.
• Organize and carry out in-home visits and chart reviews, utilizing data analysis for continuous improvement in treatment strategies.
• Ensure that treatment plans are implemented effectively and consistently across all interactions with clients.
• Engage actively in Interdisciplinary Team Meetings to discuss cases, share insights, and promote best practices in treatment.
• Document and complete monthly and quarterly progress notes to track clients' development and respond to changing needs.
• Organize training sessions for colleagues to ensure competency in program implementation and documentation.
• Conduct follow-up communications and home visits to guarantee ongoing support for clients as they navigate their mental health journeys.
• Act as a liaison with broader mental health community networks to foster collaboration and enhance services offered to clients.
• Contribute to the design of training materials to increase staff competencies and improve overall programs.

This role serves a diverse clientele, including adults and youth facing various mental health challenges. Our clients come to us seeking assistance with issues such as anxiety, depression, trauma, and substance abuse. Your empathetic approach will be vital in building rapport and trust, allowing clients to engage fully in their treatment.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related discipline from an accredited institution.
• An active and unrestricted licensure as a Professional Counselor in the state of Colorado.
• A minimum of 2 years of relevant counseling experience, preferably in a clinical setting.
• A solid understanding of legal and ethical standards governing psychological practice.
• Exceptional interpersonal and communication skills, with the ability to connect with a wide range of individuals and families.
• Demonstrated ability to work collaboratively within a healthcare team framework, showcasing commitment to holistic client care.
• Familiarity with electronic medical record (EMR) systems and other relevant technologies to document client interactions and sessions efficiently.

While not mandatory, candidates with a specialization in family therapy, trauma-informed care, or substance misuse treatment will be highly considered. The ability to leverage these specialties can further enhance our service delivery and help clients achieve their goals more effectively.

This position offers a competitive annual salary ranging from USD 80,000 to USD 105,000, commensurate with experience and caseload.
