• Conduct thorough assessments and evaluations to identify clients' social, emotional, and behavioral challenges utilizing a trauma-informed care framework.
• Develop, implement, and monitor personalized service plans tailored to the unique needs and goals of each client.
• Provide individual and family counseling sessions, employing cognitive-behavioral techniques, solution-focused therapy, and other evidence-based modalities.
• Engage clients in collaborative service planning, facilitating access to vital community resources and support networks.
• Deliver crisis intervention and risk assessments, ensuring clients’ immediate safety and well-being through effective safety planning.
