Key Responsibilities

• Conduct thorough assessments to accurately identify clients' social, emotional, and environmental needs, tailoring interventions accordingly.
• Develop personalized service plans that outline achievable goals, desired interventions, and methods for monitoring client progress.
• Provide individual and family counseling sessions, applying therapeutic techniques to facilitate healing and coping strategies.
• Collaborate with clients in creating actionable service plans while linking them to relevant community resources to fully support their needs.
• Deliver crisis intervention services, including risk assessments and safety planning, to respond promptly to emergency situations.
• Engage with various community agencies and organizations to coordinate supportive services and advocate for client needs within multiple systems.
• Participate in outreach initiatives to spread awareness about Life Line Counseling’s services, expanding access to vital resources.
• Regularly attend case conferences and necessary training to remain compliant with best practices and enhance professional development.

Patient Population / Client Focus

The client population at Life Line Counseling primarily consists of individuals dealing with substance abuse, homelessness, domestic violence, and other socioeconomic hardships. Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in Illinois is mandatory.

• Cultural competence and a solid understanding of trauma-informed care approaches are essential for this role.

Preferred Experience

Compensation and Benefits
