Position Overview

We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team at Nordic Healthcare and Mental Health Services. In this full-time remote role, you will play an essential part in supporting patients within our Hospice Center in Stamford, Connecticut, helping them navigate the emotional, social, and practical challenges associated with their health conditions. Work Environment

Key Responsibilities
  • You will conduct comprehensive psychosocial assessments through structured interviews, observational techniques, and detailed reviews of medical records to evaluate the emotional, social, and environmental factors influencing each patient's recovery.
  • Provide individualized counseling and emotional support to patients and their families, guiding them through adjustment issues related to their health challenges.
  • Collaborate with a diverse rehabilitation team to develop tailored treatment plans that incorporate each patient's psychosocial needs into their overall care approach.
  • Assist patients in identifying and accessing community resources, such as vocational rehabilitation support, housing assistance, and local support groups to enhance their post-rehabilitation journey.
  • Educate patients and families about available community resources and post-rehabilitation services, ensuring they are fully informed and empowered to make decisions about their care.
  • Participate actively in discharge planning to facilitate smooth transitions for patients moving to home or other care settings, ensuring continuity of care and support.
Patient Population / Client Focus

Your work will directly impact patients facing a range of serious health challenges, including those transitioning from acute care to hospice, individuals facing end-of-life decisions, and patients coping with chronic illnesses. You will serve as a crucial advocate, ensuring that patients' emotional and practical needs are addressed, ultimately promoting their autonomy and enhancing their quality of life.

Qualifications
  • A Master's degree in Social Work from an accredited program is required.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • A strong understanding of psychosocial aspects related to adjustment, grief, and coping strategies is essential.
  • Exceptional communication and interpersonal skills are required to effectively engage with patients and collaborate with multidisciplinary teams.
Preferred Experience
  • Previous experience in a rehabilitation or medical setting, particularly in hospice or palliative care environments, is highly desirable.
  • Familiarity with community resources and supports available to patients and families is advantageous.
  • Insight into the dynamics of team-based care in a healthcare setting, particularly concerning transitional care, is preferred.
