 Position Summary

The Telehealth Nurse provides telephonic and electronic support to patients, caregivers, and healthcare providers for prescription and specialty medications. Responsibilities include patient outreach, adherence support, clinical data collection, compliance with regulatory programs, and coordination of follow-up services.

 Key Responsibilities

  • Provide telephonic/electronic support using behavior modification and motivational interviewing to drive medication adherence
  • Collect and document patient-specific clinical information
  • Ensure compliance with regulatory, program, and service line requirements
  • Identify risks, concerns, and stakeholder needs and escalate appropriately
  • Follow up with patients and physicians to support compliance and therapy persistence
