Key Responsibilities
  • Conduct comprehensive assessments in real-time to identify patient needs and tailor individualized treatment plans.
  • Facilitate effective communication among supervisors, team members, and clients, keeping everyone informed about progress and interventions.
  • Establish an appointment schedule, coordinating home visits and necessary observations to enhance service delivery.
  • Engage in ongoing data analysis and chart reviews to assess patient outcomes and guide clinical interventions.
  • Implement and monitor program plans, ensuring that therapeutic services are delivered effectively and meet patient goals.
  • Participate in interdisciplinary team meetings to collaborate on patient care and share insights on best practices.
  • Document monthly and quarterly progress notes to track client development and adjust treatment plans as necessary.
  • Lead training sessions to ensure staff competence in program implementation and documentation processes.
