Position Overview

Hillsdale Behavioral Health Center is actively seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a remote capacity, focusing on providing outpatient services in our Hospice Center. This full-time position entails working Monday through Friday, from 8 AM to 5 PM, to support individuals and families navigating complex emotional and psychological challenges related to end-of-life care.

Key Responsibilities
  • Plan, coordinate, and implement tailored support packages to assist clients in overcoming mental health difficulties and dependencies.
  • Conduct thorough interviews and assessments to determine each client's unique strengths, needs, and challenges.
  • Develop individualized treatment plans with measurable goals that align with each client's specific situation and needs.
  • Regularly monitor and evaluate client progress, making necessary adjustments to treatment plans as requirements change.
  • Provide expert guidance and counseling during sessions, offering clients various strategies for managing their difficulties.
  • Maintain detailed and accurate records of client interactions, progress, and treatment outcomes to inform ongoing care and compliance with regulatory requirements.
  • Participate in case conferences and provide clear, evidence-based testimony when needed in court settings.
  • Work collaboratively as a key team member, cooperating with both internal staff and external community service providers.
  • Make referrals to local support services that can further assist clients in their recovery journey.

In this role, you will primarily focus on clients dealing with end-of-life issues, grief and loss, and emotional management related to critical life transitions. Qualifications
  • A Masterโ€™s degree in Social Work (MSW) from an accredited institution is required.
