This remote position will serve students and educators within the Concord, California area. Working from home offers flexibility while allowing you to engage in meaningful interactions with patients, families, and the educational community. • Independently manage a diverse caseload of students, conducting thorough assessments of their social, emotional, financial, and cultural needs to inform their treatment plans.
• Utilize a variety of therapeutic modalities and interventions, effectively addressing barriers to care to ensure students transition smoothly through various stages of support.
• Lead initiatives within the healthcare team regarding protective service issues, complex life care decisions, and development of advance directives for students requiring additional support.
• Provide crisis intervention, demonstrating sound judgment and strong clinical skills in emotionally charged situations, effectively guiding students and their families toward resources and support.
• Collaborate closely with school staff, case managers, and healthcare professionals to educate students and families, ensure they have access to vital resources and referrals, and safeguard their socioeconomic needs are incorporated into treatment plans.
• Facilitate care coordination activities, particularly for students with complicated discharge needs, ensuring that community resources are well-aligned to prevent hospital readmission.
• Engage in regular supervision and mentorship of junior social workers, interns, and volunteers, affecting department practices and improving service delivery.
• Participate in specialty team meetings, case consultations, and care conferences, enhancing communication and collaboration among varied stakeholders.
• Stay current with industry developments and maintain professionalism while providing ongoing education to peers regarding the social worker's role and psychosocial challenges that affect educational outcomes.
• Be on-call periodically to support emergency situations within the educational setting, ensuring swift responses and access to critical care.

This position focuses on children and adolescents navigating psychosocial challenges, including those with complex emotional needs, behavioral difficulties, family hardships, and socio-economic barriers. Your work will significantly contribute to their growth and stability while promoting a supportive and nurturing educational environment.

To be successful in this role, candidates must possess the following:

• A Master's degree in Social Work (MSW) from an accredited program and a current California LCSW license.
• A minimum of two years of clinical experience, preferably in a school or educational setting.
• Strong clinical assessment and intervention skills, with a demonstrated ability to advocate for patients' needs.
• Excellent communication and interpersonal skills, essential for establishing rapport with diverse populations.
• Proficiency in coordinating care across multiple disciplines and agencies.

While not mandatory, the following qualifications and experiences will be advantageous:

• Experience working in the educational sector or with adolescents and their families is highly desirable.
• Knowledge of community resources and support services available to students and families.
• Proficiency in crisis intervention strategies and strong familiarity with trauma-informed care practices.
