• Conduct comprehensive clinical assessments of patients to determine their psychosocial needs and develop individualized treatment plans.
• Utilize evidence-based therapeutic interventions tailored to the unique emotional and mental health needs of terminally ill patients and their families.
• Maintain accurate and timely clinical documentation in compliance with regulatory guidelines to ensure continuity of care.
• Collaborate with a multidisciplinary team, including physicians, nurses, and spiritual care providers, to create a holistic care strategy for patients.
• Engage in bi-weekly case consultations and peer review meetings to enhance the quality of care provided to individuals and families.
• Provide education and support to patients and families, empowering them with knowledge about mental health and available resources.
• Stay updated on the latest research developments and best practices in social work and hospice care to improve patient outcomes.

This position focuses on patients with terminal illnesses, requiring a sensitive and tailored approach to emotional and mental health care. You will work with diverse populations, including individuals facing cancer, neurologic diseases, and other life-limiting conditions. Supporting patients and their families during this critical time is paramount, as you help them cope with grief, loss, and transition.

• Master's Degree in Social Work from an accredited institution.
• Valid LCSW license in the state of Kansas.
• A minimum of 3 years of clinical experience focusing on psychotherapy and end-of-life care.
• Expertise in evidence-based, solution-focused psychotherapy interventions.
• Strong communication, collaboration, and problem-solving skills.
• Emotionally resilient and able to maintain professionalism in challenging situations.

• Experience working in a hospice or palliative care setting.
• Knowledge of grief counseling and bereavement practices.
• Familiarity with telehealth platforms and electronic medical record systems.
