As a Licensed Clinical Social Worker, your daily activities will include:

• Conducting comprehensive psychosocial assessments through interviews and observations, as well as reviewing medical records to gain insights into patients' emotional and social challenges that may affect their recovery.
• Offering counseling and emotional support to patients and their families, addressing adjustment issues, grief, and strategies for coping with the rehabilitation process.
• Collaborating with a skilled team of rehabilitation professionals to develop tailored treatment plans that consider the psychosocial dimensions of patient care.
• Assisting patients in navigating community resources, including support groups, vocational rehabilitation, and housing assistance, to encourage holistic recovery.
• Providing educational support to patients and families about post-rehabilitation services, ensuring a smooth transition back to everyday life.
• Playing an active role in discharge planning, coordinating care transitions, and advocating for patients’ needs and preferences.

Your work will focus on a diverse patient population, encompassing individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. You will address unique emotional and psychological needs while fostering resilience and strength as patients work toward regaining independence.

The ideal candidate will possess the following qualifications:

• A Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of psychosocial factors impacting recovery, including adjustment issues and coping mechanisms.
• Exceptional communication and interpersonal skills to effectively connect with patients, families, and staff.

While not mandatory, experience in a rehabilitation or medical setting is highly desirable. Candidates who have a proven track record of working with similar patient populations will bring valuable insights and enhance the quality of care provided.
