As a Clinical Social Worker, you will collaborate closely with an interdisciplinary team, including physical therapists, occupational therapists, and speech-language pathologists, ensuring a coordinated approach to patient care. The remote setting is designed to facilitate open communication and supportive teamwork, enhancing the delivery of care to patients in our hospice center.

• Conduct in-depth psychosocial assessments through interviews, observations, and the review of patient histories to determine emotional, social, and environmental factors impacting recovery.
• Provide individual and family counseling focused on adjustment issues, emotional resilience, and coping strategies, helping families navigate the complexities of end-of-life care.
• Collaborate with the rehabilitation team to create and implement individualized treatment plans that specifically address the psychosocial needs of patients.
• Facilitate connections to community resources that assist with ongoing support post-rehabilitation, including vocational rehabilitation and mental health resources.
• Educate patients and their families on available support systems and services, ensuring a smooth transition to home or alternate care settings upon discharge.
• Participate in discharge planning, contributing valuable insights into ensuring continuity of care and optimal patient outcomes.

In this role, you will work with patients who are often confronting severe health challenges due to life-limiting illnesses. The focus will be on providing empathetic support to both patients and their families, addressing not only the emotional well-being of the patients but also the contextual factors that affect their recovery and adjustment processes.

• A Master’s degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Strong understanding of psychosocial dynamics, particularly regarding adjustment issues and grief counseling.
• Outstanding communication and interpersonal skills that foster trust and rapport with patients and their families.

• Previous experience working in a rehabilitation or hospice setting is highly desirable.
• Familiarity with community resources and agencies that provide financial, emotional, and practical support to patients.
• Experience utilizing electronic health records and other technological tools to track patient progress and treatment plans.

The Clinical Social Worker position offers a competitive annual salary in the range of USD 75,000 – 100,000.
