Hope and Healing Therapy Center is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team on a full-time basis. This remote role is pivotal in addressing the psychosocial aspects of patient care within our hospice center, helping patients and their families navigate complex emotional and social challenges during critical times of rehabilitation.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments by utilizing interviews, observations, and thorough evaluations of medical records to identify emotional, social, and environmental challenges affecting patients' rehabilitation.
  • Provide individualized counseling and support services to patients and their families, focusing on coping strategies, adjustment issues, and emotional well-being as they undergo rehabilitation.
  • Collaborate closely with a multidisciplinary rehabilitation team to create and refine tailored treatment plans that align with patients' psychosocial needs and overall rehabilitation goals.
  • Guide patients in accessing vital community resources, such as support networks, vocational rehabilitation services, and housing assistance, to ensure a holistic approach to their recovery.
  • Educate and support patients and their families regarding available community resources and post-rehabilitation services for a smooth transition back to independent living.
  • Participate actively in discharge planning, facilitating transitions to home care or alternative care settings while ensuring ongoing support for patients and their families.
Patient Population / Client Focus

Qualifications

Compensation and Benefits
