Key Responsibilities
  • Conduct comprehensive and ongoing assessments of clientsโ€™ mental health needs to tailor personalized treatment plans.

  • Provide a well-structured schedule of appointments and coordinate necessary home visits while documenting observations for continuous improvement.
  • Ensure the effective delivery and monitoring of treatment plans, actively participating in interdisciplinary team meetings to review and refine strategies.
  • Document monthly and quarterly progress notes to track treatment efficacy and to inform further interventions.
  • Organize training and mentorship for team members concerning the implementation of therapy techniques and record-keeping protocols.
  • Act as a liaison between clients, their families, and the broader mental health community to aid in developing tailored treatment goals and resources.
  • Oversee follow-up interviews and home visits to ensure ongoing support aligned with client needs.
  • Utilize electronic medical record (EMR) systems effectively to document all patient interactions and maintain accurate records.
Patient Population / Client Focus

Your role will encompass working with a diverse client population including adults and youth who present various mental health challenges. Your expertise will be crucial in addressing issues such as anxiety, depression, trauma, and relationship difficulties, particularly focusing on family and couples therapy.

Qualifications
  • A Masterโ€™s or Doctoral degree in Counseling, Psychology, or a closely related field is required.
  • Active and unrestricted licensure as a Professional Counselor (LPC) in Oregon.
