• Effectively manage a caseload of diverse patients, conducting thorough evaluations to identify social, emotional, financial, and cultural factors impacting their health and treatment plans.
• Lead interdisciplinary care teams to navigate complex psychosocial issues, advocating for high-risk patients, and actively participating in treatment planning.
• Provide crisis intervention and support to patients and their families, facilitating immediate solutions to urgent issues.
• Coordinate with healthcare providers, ensuring that psychosocial and socioeconomic needs are systematically integrated into care protocols and discharge plans.
• Deliver ongoing education to your healthcare team about the critical role of social work in patient care and the psychosocial factors that influence treatment outcomes.
• Supervise and mentor junior staff, interns, and volunteers, fostering a supportive learning environment while adhering to departmental and practice standards.
• Engage with community agencies and resources that can aid in preventing patient readmissions, optimizing the utilization of available funds and services.
• Ensure timely completion of psychosocial services to prevent delays in treatment implementation and patient discharge timelines.
• Participate actively in hospital rounds and care conferences to contribute insights based on your caseload and expertise in social work.
• Respond promptly to any crisis that may arise, supporting both staff and patients through complex emotional challenges.

Understanding the emotional landscape of patients in hospice care will be crucial, as you guide families through end-of-life decision-making and support them in navigating grief and loss during these challenging times.

• A Bachelor’s Degree in Social Work from an accredited institution.
• Current LCSW licensure in California.
• A minimum of two years of prior experience in a clinical social work setting, preferably with hospice or palliative care exposure.
• Excellent communication and interpersonal skills, with a strong ability to work effectively in a multidisciplinary team.

• Experience working with patients in hospice care or related fields.
• Familiarity with case management, crisis intervention, and community resource coordination.
• Advanced knowledge of ethical and legal standards in social work practice.

This position offers a competitive salary ranging from USD 120,000 to 130,000 annually.
