Serene Sanctuary Counseling, LLC is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time capacity. As a remote social worker, you will play a vital role in supporting patients and their families in a hospice setting, providing them with continuous and culturally sensitive mental health services aimed at enhancing their quality of life during challenging times.

This position is entirely remote, allowing for flexible work hours and a comfortable work-life balance. While working remotely, you will have the liberty to set your schedule, making it easier to accommodate the needs of your patients and personal commitments. You will join a close-knit and collaborative multidisciplinary team, engaging with psychiatrists, psychologists, nurses, and case managers to create a comprehensive care plan for each patient.

• Conduct comprehensive mental health assessments including psychosocial evaluations, diagnostic interviews, and risk assessments tailored specifically for hospice clients.
• Collaborate with patients and their families to create individualized treatment plans that are culturally responsive and designed to meet their unique needs and goals.
• Implement evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) across various settings including individual, couple, family, and group therapy.
• Provide urgent crisis intervention services, working closely with other healthcare professionals to ensure a seamless continuity of care for patients experiencing acute distress.
• Maintain thorough and accurate clinical documentation, including treatment plans, progress notes, and discharge summaries, in accordance with state regulations and clinic guidelines.
• Empower patients and their families through psychoeducation, equipping them with the knowledge and skills necessary to manage their mental health effectively.
• Engage in regular outcome assessments to evaluate the effectiveness of treatment and make timely adjustments to care plans as required.
• Stay abreast of current research, best practices in mental health care, and emerging therapeutic trends, applying this knowledge to enhance clinical practice.
• Adhere to all ethical guidelines and maintain confidentiality in handling patient information at all times.

As an LCSW, you will primarily focus on a diverse patient population within a hospice care setting. This includes working with patients facing terminal illnesses and their families, navigating the emotional challenges that accompany life-limiting conditions. Your services will not only be therapeutic; they will also encompass grief counseling, bereavement support, and psychoeducation, ensuring that families are adequately supported as they face difficult transitions.

• A Master’s degree in Social Work is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) is mandatory.
• The ability to work effectively both independently and collaboratively within a multidisciplinary team is essential.
• Exceptional interpersonal and communication skills with a demonstrated ability to establish rapport with patients from a variety of cultural backgrounds.

• Experience working within hospice or palliative care settings is highly desirable.
• Familiarity with evidence-based therapeutic techniques and outcome measures in mental health.
• A strong background in crisis intervention and trauma-informed care.
• Knowledge of legal and ethical guidelines surrounding social work practices, particularly in healthcare.
