Key Responsibilities
  • As a Licensed Clinical Social Worker reporting to the Social Work Manager, you will conduct thorough evaluations, diagnosing and treating psychosocial issues that hinder patients' ability to achieve their highest potential.
  • You will uphold the professional standards of social work, strictly adhering to the NASW Code of Ethics in all interactions and interventions.
  • Develop and implement individualized treatment plans utilizing both individual and group therapy modalities, focusing on fostering therapeutic relationships and empowering patient autonomy.
  • Maintain accurate documentation of patient progress and interventions in compliance with departmental standards and regulations.
  • Collaborate closely with medical staff and support teams to create comprehensive care plans that enhance treatment adherence and optimize patient outcomes.
  • Provide education and resources to patients and families, helping them understand and implement recommendations to improve their psychosocial functioning.
  • Engage in continuous education and professional development through training opportunities to enhance clinical skills.
  • Monitor community resources and advocacy needs, guiding patients in accessing support networks that bolster their social and emotional health.
Patient Population / Client Focus

Your primary focus will be on serving adults experiencing a variety of mental health challenges, including but not limited to anxiety, depression, trauma, and stress-related issues. You will work to address the psychosocial needs of these individuals, ensuring they receive the support necessary to navigate their conditions effectively.

Qualifications
  • A Masterโ€™s Degree in Social Work from an accredited institution and a current LCSW license issued by the State Board of Social Work Licensure.
