As a remote Social Worker at Well Spring Healthcare Services, you will have the flexibility to manage your schedule while actively engaging with clients from the comfort of your home office. The dynamic detox center setting will require you to utilize technology for client assessments and treatment planning, ensuring continuity of care. In your role as an LCSW, you will:

• Conduct comprehensive assessments of clients’ psychosocial needs, with a focus on emotional, mental, and social factors, ensuring holistic care.
• Develop and implement individualized care plans in collaboration with clients and their families, addressing their specific goals and needs.
• Provide emotional support and counseling to clients undergoing detox, assisting them in navigating the challenges associated with recovery.
• Advocate for clients' rights by connecting them with essential community resources, ensuring they receive comprehensive support and services.
• Collaborate closely with a multidisciplinary team, including medical professionals, caregivers, and case managers, to coordinate care effectively.
• Regularly monitor client progress through virtual check-ins, adjusting care plans as necessary to ensure optimal outcomes.
• Facilitate constructive communication and mediation among family members involved in the care of clients, fostering a team-oriented approach.
• Educate clients and families regarding available resources, support groups, and community programs to enhance social support and overall quality of life.
• Maintain accurate and timely documentation of client interactions, assessments, and care plans to comply with legal and ethical standards.
• Stay updated on the latest developments in social work practices, policies, and regulations to provide the highest standard of care.

The primary focus of this role will be on individuals navigating the challenges of detoxification and recovery. You will engage with a diverse range of clients, including seniors, who often face unique emotional hurdles. Your expertise will help empower them to manage anxiety, depression, and the overall impacts of substance use in their lives, ensuring they have the tools necessary for sustained recovery.

• A Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• At least two years of relevant social work experience, with a preference for exposure to community health or geriatric settings.
• Thorough understanding of social work principles and aging-related issues, paired with knowledge of community resources available for clients.
• Strong assessment, counseling, and critical thinking skills, enabling you to tailor interventions to the unique needs of each client.
• Ability to empathize and establish meaningful connections with clients and families.
• Excellent communication and interpersonal skills that facilitate effective collaboration within interdisciplinary teams.
• Proficiency with electronic health record (EHR) systems and relevant computer applications.

While a minimum of two years of social work experience is required, candidates with specialized experience in substance use treatment, geriatric care, or mental health advocacy will find themselves particularly well-suited for this role. Familiarity with detox environments or community resource networks will be highly advantageous.
