• Conduct thorough assessments that evaluate clients' social, emotional, and environmental challenges, identifying their specific needs and obstacles.
• Develop personalized service plans that include defined objectives, intervention strategies, and ongoing progress monitoring to ensure client success.
• Provide compassionate counseling to individuals and families facing a multitude of life challenges, focusing on mental health, emotional resilience, and coping strategies.
• Collaborate with clients to create tailored service plans and connect them to community resources, ensuring they receive well-rounded support.
• Implement crisis intervention and risk assessment protocols, creating safety plans when necessary to protect clients' well-being.
• Foster collaboration with community organizations and agencies to coordinate services and effectively advocate for clients' needs.
• Engage actively in community outreach initiatives to raise awareness and promote the valuable services provided by Health Center Care Plans Inc.
• Participate in regular case conferences, multidisciplinary team meetings, and ongoing training sessions to enhance skill sets and foster a cohesive team environment.

Your primary focus will be individuals engaged in detoxification processes seeking recovery from substance abuse issues. These clients often face complex challenges, including mental health disorders, trauma histories, and socio-economic barriers. As part of your role, you will utilize trauma-informed care principles to ensure that each interaction is respectful, supportive, and tailored to fit clients' individual needs, ultimately facilitating their journey toward recovery and stability.

• A Master’s degree in Social Work from an accredited institution is required to ensure foundational knowledge and skills.
• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW) is mandatory for clinical practice.
• This position requires strong knowledge of social work principles, social and economic justice issues, and community resources relevant to our client base.
• Excellent communication skills, cultural competence, and problem-solving abilities are essential for effectively supporting our diverse client population.

• Previous experience working in a social service agency, particularly in addiction treatment or mental health settings, is highly preferred.
