While our LCSWs primarily operate from home, they will engage frequently with patients and team members through secure telehealth platforms. • Provide comprehensive therapeutic services, including psychotherapy, case management, and intensive counseling to clients, always adhering to Evidence-Based Practices (EBPs).
• Conduct thorough mental status examinations, promptly identifying changes in clients' mental health conditions and adjusting treatment strategies as necessary.
• Document client interactions meticulously through clear, concise case notes, progress reports, and treatment plans that comply with professional and regulatory standards.
• Utilize empathy-driven approaches in challenging situations to de-escalate stress, and guide clients through emotional difficulties while offering actionable solutions.
• Collaborate effectively with other healthcare professionals, including psychiatrists, case managers, and community resources, to coordinate holistic care for each patient.
• Facilitate therapeutic interventions in various settings, tailoring approaches for both individual and group therapy depending on client needs and preferences.
• Actively contribute to the professional development of team members by sharing clinical insights, best practices, and collaborative strategies.

You will primarily support vulnerable individuals and families navigating the complexities of end-of-life care and bereavement within a hospice environment. This significant work requires not just clinical skills but also a deep sense of compassion and cultural sensitivity to honor diverse family dynamics and individual client experiences.

• A Master’s degree in Social Work from an accredited program.
• A valid Virginia license as a Clinical Social Worker (LCSW), with the capability to practice independently.
• A high level of emotional intelligence, allowing for strong rapport-building and a trusting relationship with clients.
• Proven experience in therapeutic settings, particularly with individuals, couples, and families in crisis.
• Demonstrable organizational and time-management skills, ensuring responsive client care and adherence to clinical deadlines.
• Exceptional verbal and written communication skills necessary for effective collaboration with team members and clear documentation.

• Experience in hospice or palliative care settings, understanding the unique emotional landscapes of end-of-life scenarios.
• Familiarity with electronic health record (EHR) systems and telehealth platforms to enhance remote service delivery.
• Strong background in utilizing treatment modalities such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), or other relevant techniques.
