Key Responsibilities

  • Develop and implement individualized intervention and discharge plans designed to address the unique needs of each patient, promoting effective coping strategies for hospitalization, illness, or significant life changes.
  • Engage in crisis intervention when necessary, employing both brief and long-term therapeutic approaches to provide immediate and ongoing support.
  • Collaborate closely with multidisciplinary healthcare teams, including physicians, nurses, and other specialists, to coordinate patient care and involve patients and families in their treatment plans.
  • Facilitate educational workshops or training for colleagues and other healthcare professionals concerning psychosocial aspects and interventions relevant to patient care.
  • Participate in discharge planning, working alongside third-party payers and healthcare providers to ensure a smooth transition for patients leaving the care facility.
  • Evaluate oncology patients for mental health issues such as depression and anxiety, delivering short-term counseling as required.
  • Contribute to community services aimed at enhancing support for cancer survivors and promote education about available resources.
Patient Population / Client Focus

This position focuses on a diverse patient population, including those dealing with chronic illnesses, mental health challenges, and psychosocial issues related to hospitalization. Particular emphasis will be placed on serving patients within the field of oncology, facilitating their emotional and psychological adjustment to diagnosis and treatment processes.

Qualifications

The ideal candidate will possess a Bachelor’s Degree in Social Work, along with a current license to practice as a Licensed Clinical Social Worker (LCSW). A strong foundational knowledge in mental health principles, psychosocial assessments, and therapeutic interventions is essential.

Preferred Experience
