We deliver individualized, family-centered support - partnering with families and caregivers and applying an interdisciplinary approach so every patient can reach their fullest potential.
The Role
We're looking for a Registered Nurse (RN) - Virtual to provide remote clinical oversight, telephonic triage, and care coordination. In this 100% remote role, you will perform clinical assessments, monitor patient treatment plans, and collaborate with physicians and interdisciplinary teams to ensure quality care and regulatory compliance.
What You'll Do
  • Conduct virtual clinical intakes, health screenings, and wellness checks with patients and caregivers.
  • Perform telephonic triage and clinical assessments for patient calls, directing care using established protocols.
  • Review and update care plans in alignment with physician orders and state regulations.
  • Monitor Remote Patient Monitoring (RPM) data and vital trends, flagging clinical changes for the care team.
  • Serve as a primary virtual point of contact for patients, families, attending physicians, and home care staff.
  • Maintain accurate, timely clinical documentation in our EHR platforms.
  • Participate in virtual case conferences, team huddles, and quality assurance reviews.

What You Bring
  • Active, unencumbered Registered Nurse (RN) license in the state of New Jersey required (Multi-state Compact Licensure preferred).
  • Minimum of 1-2 years of clinical experience in home health, triage, or remote care management.
  • Strong clinical triage, assessment, and critical-thinking skills.
  • Professional, empathetic communication skills across phone and video channels.
