Job description

Neuron Health Systems Speech-Language Pathologist

Neuron Health Systems is a community-based provider of acute outpatient rehabilitation and medical and non-medical direct care. We deliver individualized, family-centered support — partnering with families and caregivers and applying an interdisciplinary approach so every patient can reach their fullest potential.

We're looking for a Speech-Language Pathologist to support patients with communication, cognitive, and swallowing needs in their homes and communities. This is a hybrid, per-diem role: most planning and documentation is done remotely, with in-person time reserved for evaluations and hands-on treatment sessions.

All clinicians are expected to take part in the firm's accreditation activities and surveys as part of maintaining our quality standards.

What You'll Do

  • Conduct comprehensive speech, language, cognitive, and swallowing evaluations.
  • Develop and implement individualized treatment plans.
  • Provide dysphagia management and swallowing safety recommendations.
  • Monitor patient progress and adjust treatment plans as needed.
  • Collaborate with RNs, PTs, OTs, and other members of the care team.
  • Complete evaluation, progress, and discharge documentation in a timely manner.
  • Educate patients and family members on communication strategies and home programs.
  • Participate in case conferences and firm accreditation activities and surveys.

What You Bring

  • Active NJ Speech-Language Pathologist license (or CFY candidate) in good standing.
  • Home care, rehabilitation, or I/DD experience preferred.
  • Comfort working independently in a remote-first workflow, with reliable transportation for in-person visits.
  • Strong documentation habits and clear communication with patients, families, and the care team.
