Clear Mindcare Counseling Services, LLC is seeking a dedicated Remote Licensed Mental Health Therapist, holding a current LCSW, LMFT, or LPC license, to join our dynamic team in Sandy, Oregon. • Complete same-day documentation for each participant identified as being at increased risk of utilizing crisis services, ensuring compliance and timely communication.
• Respond promptly to critical incidents by completing reporting forms within 24 hours to ensure that all necessary parties are informed and involved.
• Participate in after-hours phone support on an alternating schedule to provide care coordination for urgent situations and ongoing patient support.
• Collaborate proactively with clients to mitigate the need for crisis interventions, focusing on early identification of potential triggers and symptoms.
• Maintain accurate crisis alerts and coordinate efforts with county crisis services to streamline responses for participants.
• Facilitate smooth transitions for clients moving from hospital settings to outpatient care, ensuring thorough communication and handoffs with hospital staff.
• Employ de-escalation techniques and provide milieu support as the Clinician of the Day, addressing immediate mental health concerns effectively.
• Conduct comprehensive risk assessments and develop individualized safety plans to enhance participant stability.

Our therapists serve a diverse client base within the community, focusing on individuals experiencing a range of mental health challenges. This may include anxiety disorders, depression, trauma-related conditions, and more severe psychological issues. By fostering a trusting therapeutic alliance, we aim to empower clients to engage actively in their healing process.

• A current independent license in the state of Oregon (such as LCSW, LMFT, or LPC) is required.
• A Master's or Doctorate degree in clinical psychology, counseling, or clinical social work from an accredited institution is mandatory.
• Excellent communication skills, with the ability to articulate complex ideas and maintain professionalism under stress.
• Competence in crisis management and risk assessment, coupled with strong analytical capabilities.

• Prior experience in a community mental health setting and familiarity with electronic health records (EHR) systems.
• Experience dealing with high caseloads in fast-paced environments while ensuring quality of care.
• Demonstrated ability to motivate and inspire clients, promoting resilience and self-efficacy.
