Key Responsibilities
  • Complete same-day documentation for each participant identified as being at increased risk of utilizing crisis services, ensuring compliance and timely communication.
  • Respond promptly to critical incidents by completing reporting forms within 24 hours to ensure that all necessary parties are informed and involved.
  • Participate in after-hours phone support on an alternating schedule to provide care coordination for urgent situations and ongoing patient support.
  • Collaborate proactively with clients to mitigate the need for crisis interventions, focusing on early identification of potential triggers and symptoms.
  • Maintain accurate crisis alerts and coordinate efforts with county crisis services to streamline responses for participants.
  • Facilitate smooth transitions for clients moving from hospital settings to outpatient care, ensuring thorough communication and handoffs with hospital staff.
  • Employ de-escalation techniques and provide milieu support as the Clinician of the Day, addressing immediate mental health concerns effectively.
  • Conduct comprehensive risk assessments and develop individualized safety plans to enhance participant stability.
Patient Population / Client Focus

Our therapists serve a diverse client base within the community, focusing on individuals experiencing a range of mental health challenges. This may include anxiety disorders, depression, trauma-related conditions, and more severe psychological issues. Qualifications
  • A current independent license in the state of Oregon (such as LCSW, LMFT, or LPC) is required.
