• Provide comprehensive social work services to clients in hospice care from a remote setting, offering emotional support and practical assistance.
• Conduct thorough assessments of patients’ needs, formulate individualized treatment plans, and monitor progress against defined goals.
• Develop and implement customized care strategies that cater to the unique situations of each client, ensuring a focus on their emotional well-being.
• Advocate for clients’ rights, helping them access necessary resources and ensuring they receive the care that respects their dignity.
• Collaborate with healthcare providers, families, and interdisciplinary teams to coordinate care, share insights, and improve patient outcomes.
• Maintain detailed documentation of client interactions and care plans in compliance with legal and ethical standards.

Your role will primarily involve working with clients who are experiencing end-of-life transitions. This sensitive coverage requires a deep understanding of grief and loss and the ability to provide compassionate support to individuals and families. • Master's degree in Social Work (MSW) from an accredited institution.
• Must be a Licensed Clinical Social Worker (LCSW) in Oregon, demonstrating your qualifications to practice independently.
• At least 3 years of experience in social work, particularly within a hospice or healthcare setting, is essential.
• Exemplary communication, empathy, and problem-solving skills are critical for effective interaction with clients and families.
• Comfort using various remote work technologies, including telehealth platforms and electronic health records (EHR).
