Job Description Summary

We are seeking a bilingual (English and Spanish) highly skilled and empathetic Advanced Practice Registered Nurse (APRN) to join our telehealth team. This specialized role focuses on providing comprehensive memory assessment and care through a virtual platform. You will be responsible for conducting cognitive screenings managing the symptoms of patients living with dementia.
A portion of this role involves managing the complex behavioral and psychological symptoms of dementia (BPSD), ensuring that both patients and their caregivers have a clear, medically sound path forward from the comfort of their homes.

Key Responsibilities

  • Virtual Assessments: Conduct comprehensive telehealth evaluations for patients with memory concerns, including detailed clinical histories and remote physical/neurological observations.

  • Cognitive Screening: Administer and interpret validated remote assessment tools (e.g., Mini-Cog, MoCA, SLUMS) todeterminethe stage and type of cognitive impairment.

  • Psychiatric Management: Diagnose and manage neuropsychiatric complications such as agitation, anxiety, depression, and sleep disturbances using both pharmacological and non-pharmacological interventions.

  • Caregiver Support: Educate families on disease progression, safety planning (wandering, driving), and behavioral de-escalation techniques.

  • Documentation:Maintainelectronic health records (EHR) in compliance with Florida state regulations and HIPAA standards for telehealth.

Qualifications & Requirements

  • Licensure: Must hold a current, unencumbered Florida APRN license with prescriptive authority.
