About the Role

This is a unique opportunity for a clinically experienced LVN or RN to step into a high-impact operational role at the agency level. As our Weekend Clinical Intake Coordinator, you will serve as the central point of contact for our entire organization on Saturdays and Sundays, managing Kaiser referral intake, supporting field clinicians with clinical escalations, and ensuring our patients receive timely, seamless care coordination when our weekday leadership team is offline.

This role is ideal for a clinician with home health experience who thrives in a fast-paced, autonomous environment, communicates confidently with both clinical and operational partners, and can triage competing priorities with clarity and calm.

What You'll Do

Kaiser Referral Intake & Coordination

  • Serve as 21HHS's dedicated point of contact for Kaiser on weekends, managing all incoming referral requests and acceptance confirmations
  • Communicate directly with Kaiser representatives to confirm referral volumes, acceptance windows, and scheduling timelines
  • Ensure all referrals meet intake criteria and are entered and assigned accurately and on time
  • Track and manage Kaiser's weekend referral demands, ensuring the agency meets agreed-upon response expectations

Field Clinician Support & Schedule Management

  • Assist field nurses and clinicians with urgent patient reassignments and schedule gaps throughout the weekend
  • Triage incoming calls from the field, helping clinicians prioritize and navigate unexpected patient needs
  • Coordinate open capacity across the field team in real time, matching available clinicians to urgent patient needs
  • Serve as the first point of escalation for clinical questions, supporting staff in the same capacity a Clinical Supervisor would during the week

Clinical Escalation Management

  • Triage and respond to clinical escalation calls from field staff, patients, and Kaiser throughout the weekend
  • Apply clinical judgment to de-escalate, redirect, or elevate issues as appropriate
  • Maintain clear documentation of all escalations, actions taken, and outcomes

        Support staff with questions about clinical protocols, patient care               scenarios, and next-step guidance

What We are Looking For
    • Active California LVN or RN license in good standing
    • Minimum 2 years of clinical experience; home health experience strongly preferred
    • Prior intake, scheduling, or care coordination experience preferred
    • Strong clinical triage skills - comfortable making judgment calls independently
    • Excellent verbal communication skills; confident and clear when speaking with Kaiser representatives, clinical staff, and leadership
    • Highly organized with the ability to manage multiple calls, tasks, and priorities simultaneously in a variable-volume environment
    • Comfortable with ambiguity - some weekends will be high-volume, others will be light
    • Proficiency with CareStitch, Kinser, or similar home health platforms preferred
    • Valid California driver's license with acceptable driving record
    • All licensure and certifications must be current at time of hire and maintained throughout employment
