Job Title: OPWDD Care Manager (Remote)

Pay: $50k-$75k | Location: Remote, NY travel 1x/quarter | Type: Full-time

What you’ll do as an OPWDD Care Manager:

Coordinate services for adults with I/DD. Manage a 35-40 person caseload: build Life Plans, do home visits, handle crises, and keep families + providers aligned. 40% documentation in EHR. You’re the hub for medical, behavioral, and social services.

You’re our OPWDD Care Manager if you have ONE of these:

Bachelor’s + 2 yrs relevant experience OR

RN + 2 yrs experience OR

Master’s + 1 yr experience OR

Were an MSC before 7/1/2018

Must have: OPWDD experience, strong de-escalation skills, HIPAA knowledge, Microsoft Suite. You close loops and don’t drop paperwork.
