Are you an experienced Registered Nurse with a background in home health who enjoys working independently and making a meaningful impact in the community? This long-term opportunity as a Home Health Nurse Assessor allows you to conduct in-home assessments that help ensure patients receive the care and services they need.

Position Overview

  • Position: Registered Nurse (RN) – Home Health Nurse Assessor

  • Specialty: UAS Assessment / Home Health

  • Assignment Length: Long-Term Assignment

  • Location: Upper East / West Side


Minimum Requirements

Candidates must have at least 2 years of home-based healthcare experience within the last 10 years, including one or more of the following:

  • Field work with a Certified Home Health Agency (CHHA)

  • Work with a Licensed Home Care Services Agency (LHCSA)

  • Experience with Managed Long-Term Care (MLTC) programs

  • Conducting home-based patient assessments (e.g., OASIS)

  • Providing primary care to homebound patients

  • Care management for individuals receiving home and community-based services, hospice, or assisted living

  • Home care experience as an LPN or Home Health Aide caring for at least 2 patients per week
