Job description

Salary: $55-$120 per hour

We are specifically looking for Registered Nurse (RN) with a background inhome health care experience. Applicants must have prior experience in home health nursing to be considered for this role.


Our agency Evolve Home Health is located in Agoura Hills, CA, we provide services throughout Los Angeles County and extending into the Mid/Central Valley. (Fresno, Clovis, Madera, Coalinga, Mendota, Merced, Tulare, Visalia, San Diego area, and Los Angeles County)



ESSENTIAL FUNCTIONS

  • Provide professional nursing care, go to the patients home to do admission, and ongoing assessments to patients
  • Initiate, develop and update individual appropriate plan of care according to an Individual patients needs
  • Submit all nurses notes on a timely manner into the system to be in compliance with HIPPA regulations, maintain accurate records
  • Administer medications and assist with pain management
  • Clean and dress wounds
  • Document symptoms and vital signs
  • Monitor patient health and update care plan accordingly
  • Instruct patients and their families on proper home care
  • Provide suggestions to improve safety at home
  • Detect early symptoms that could lead to a hospital visit
  • Supervise home health aides
  • Exceptional communication with patients, staff, physicians, social workers or other health advisors
  • Provide encouragement and support


SKILLS

  • Proficiency in using electronic health record (EHR) systems (e.g., DataSoftLogic)
  • Familiarity with durable medical equipment (DME) and its use in patient care
  • Proficiency in OASIS nursing documentation to ensure completeness, timeliness, and adherence to agency policies and Medicare guidelines. (training provided if needed)

  • Physical Exam, TB Test, Chest X-ray
