As an LCSW specializing in hospice care, you will be entrusted with a variety of important responsibilities, including:

• Conducting thorough clinical assessments to determine the needs of individuals and families facing end-of-life challenges, thereby developing comprehensive, personalized treatment plans that incorporate evidence-based practices.
• Delivering specialized psychotherapy services to patients, utilizing a range of therapeutic modalities, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), while adapting approaches based on client preferences and circumstances.
• Maintaining meticulous clinical documentation to ensure compliance with HIPAA and other pertinent regulations, while also leveraging electronic health record systems to streamline data management and improve patient care.
• Collaborating closely with an interdisciplinary team of professionals, including psychiatrists, nurses, and other social workers, to coordinate comprehensive care and address the multifaceted needs of clients and their families.
• Facilitating regular case consultations and participating in peer review meetings to foster continuous improvement and integrate new insights into practice.
• Providing ongoing education and support to patients and families regarding mental health issues, treatment modalities, and community resources, offering a holistic approach to tackle emotional and psychological challenges.
• Staying informed about current research, methodologies, and innovations within mental health and hospice care, actively seeking integration into your therapeutic practices.

The primary focus of your work will be with patients in hospice care, an emotionally sensitive environment requiring a deep understanding of the psychological impacts of terminal illness. You will engage with individuals facing serious health conditions and support their families through grief, terminal care planning, and emotional resilience building. • A Master’s degree in Social Work from an accredited institution is mandatory.
• A current and valid license to practice as a Licensed Clinical Social Worker (LCSW) in California is required.

• Strong foundation in solution-based therapeutic interventions, along with excellent communication and interpersonal skills.

• The salary for this position ranges from USD 115,000 to 130,000 annually, reflecting your skills and experience.
