We are hiring for:
CAP Nurse / PT / Buncombe & Madison Counties / Flexible Hours
Type:

Provide basic case management services to medically fragile children who are at risk of institutionalization and whose health, safety, and well-being can be maintained in his/her primary residence. Coordinate services with RN to ensure quality care.
Responsibilities include:
  • Establish eligibility for services, obtain client histories, perform initial assessments, develop a plan to provide basic services, explain available services and programs, and refer clients to appropriate programs/agencies for other needed services.
  • Evaluate and document client programs
  • Conduct interviews and work closely with clients, families, other relatives, school, medical providers, and other community agencies contacts to obtain information to be incorporated into a social history which will aid in planning for required services as appropriate to the individual plan of care.
  • Provide basic case management services as the lead member of a Social Worker/RN interdisciplinary team.

Requirements:
  • Graduation from an accredited School of Nursing. Ability to direct and carry out prescribed medical treatments and therapeutic programs.
  • Thorough knowledge of professional nursing theory and practice. RN certification with one year of nursing experience preferred.

Required Licensure/Certifications
  • Valid NC driver's license and a clean driving record (this is a condition of employment) Personal auto liability insurance is mandatory if transporting clients in private/personal vehicle(s). Must meet all other conditions of employment related to criminal record screening, drug screening, reference checking, etc.

  • Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Drug testing
  • Education verification and other credentialing based on position requirements.

  • Positions that require driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.
