This role is entirely remote, allowing you the flexibility to provide support from your own safe and private space. You will connect with patients and families through virtual platforms, ensuring they receive high-quality mental health care in the comfort of their own homes. Working with a multidisciplinary team, you will collaborate with psychiatrists, nurses, and other healthcare professionals to ensure comprehensive care.

• Conduct thorough mental health evaluations, including psychosocial assessments, diagnostic interviews, and risk assessments to ascertain the needs of patients.
• Design and implement individualized treatment plans tailored to the unique goals and cultural considerations of each patient.
• Deliver evidence-based therapies, such as CBT, DBT, and solution-focused therapy, in individual, couple, family, and group therapy settings.
• Facilitate crisis intervention services, coordinating care with medical and psychiatric providers to ensure timely and effective treatment for patients in distress.
• Maintain detailed and accurate clinical documentation, including progress notes, treatment plans, and discharge summaries, in compliance with regulatory standards.
• Engage patients and their families in psychoeducation, equipping them with the tools and knowledge necessary for managing mental health conditions.
• Collaborate with a multidisciplinary team to optimize patient outcomes, navigating challenges and facilitating seamless service transitions.
• Evaluate and monitor treatment effectiveness through ongoing assessments and adapt plans as needed for improved outcomes.
• Stay informed on the latest research and trends in mental health care to enhance clinical practice and service delivery.
• Adhere strictly to ethical guidelines, maintaining the confidentiality and sensitive information of all patients.

Your primary focus will be on hospice patients, who often face complex emotional and psychological challenges associated with end-of-life issues. You will be instrumental in providing empathetic support, assisting both patients and their families in navigating the emotional terrain of grief, loss, and acceptance. Your work will contribute significantly to enhancing their quality of life during this critical time.

• A Master's degree in Social Work from an accredited institution is required.
• An active State license as a Licensed Clinical Social Worker (LCSW) is essential.
• Demonstrated ability to work both independently and collaboratively within a multidisciplinary team environment.
• Exceptional interpersonal and communication skills, with the ability to establish rapport with patients and families from diverse backgrounds.

Experience in hospice or palliative care settings is preferred but not required. Familiarity with trauma-informed care practices and proficiency in evidence-based treatment modalities will be valuable assets. Previous experience working with diverse populations and understanding their unique cultural perspectives will enhance your effectiveness in this role.

The annual salary for this position ranges from USD 80,000 to 100,000, commensurate with experience and qualifications. Additional benefits include:
