• Conduct thorough clinical assessments to determine patient needs and formulate comprehensive treatment plans tailored to support their mental, emotional, and social well-being.

• Implement evidence-based therapeutic interventions, utilizing various methodologies to address diverse needs, ensuring that patients receive the best possible care.

• Keep meticulous records of clinical sessions, treatment updates, and patient progress while adhering to all confidentiality and regulatory standards.

This role primarily focuses on patients experiencing complex emotional and psychological challenges associated with end-of-life care. As an LCSW, you will support individuals dealing with grief, anxiety, and adjustment disorders, ensuring they receive holistic care. Your role will also extend to family members, assisting them with coping strategies and resources to navigate these difficult transitions.

• A Master’s degree in Social Work from an accredited institution.

• A valid Licensed Clinical Social Worker license in the state of Texas.

• Proficiency in solution-based psychotherapy interventions, with a deep understanding of grief and bereavement counseling.

• Exceptional interpersonal communication skills and a proactive problem-solving attitude.

• Experience working within a hospice or palliative care setting is highly desirable.

• Familiarity with electronic health record (EHR) systems and other telehealth tools to facilitate effective patient interactions.

• Demonstrated ability to work collaboratively in a multidisciplinary team environment.

• Competitive salary range of USD 80,000 to 104,000 per year, commensurate with experience and qualifications.
