Position Summary

The Registered Nurse (RN) is responsible for providing and coordinating skilled nursing care for patients in the home setting. The RN completes comprehensive assessments, develops and implements patient care plans, and ensures care is delivered in accordance with physician orders, Medicare Conditions of Participation, state regulations, and agency policies.

The RN serves as a key member of the interdisciplinary team and ensures safe, effective, and compliant care delivery.

Reporting Structure

This position reports to the Director of Nursing (DON) or designated clinical leadership. The RN functions as part of the interdisciplinary care team and provides supervision and direction to licensed and unlicensed clinical staff as applicable.

Scope of Practice

The RN performs nursing services within the scope of Florida RN licensure, federal and state regulations, and agency policies. The RN is responsible for clinical judgment, nursing assessments, care planning, and coordination of patient care in the home health setting.

Essential Job Functions

1. Patient Assessment & Care Planning

  • Perform comprehensive and ongoing patient assessments
  • The RN participates in start of care (SOC), recertification, and reassessment visits as required.
  • Complete OASIS assessments as required
  • Develop, implement, and update individualized patient care plans
  • Evaluate patient response to care and adjust interventions as needed
  • Ensures physician orders are appropriate and consistent with the patient’s clinical condition and care plan, and communicates discrepancies as needed.

2. Skilled Nursing Care & Coordination

  • Provide skilled nursing services in accordance with physician orders and care plans
  • Coordinate care with physicians, therapists, and interdisciplinary team members
  • Ensure continuity of care across all services provided

3. Monitoring & Clinical Decision Support

  • Monitor patient condition and identify changes in physical, emotional, or functional status
  • Communicate significant changes promptly to physician and care team
  • Implement appropriate interventions within scope of practice

4. Documentation & Compliance

  • Complete accurate, timely clinical documentation in the EMR system
  • Ensure documentation supports medical necessity and compliance requirements
  • Maintain adherence to Medicare Conditions of Participation and agency policies
  • Completes OASIS assessments accurately and within required timeframes in accordance with Medicare regulations and agency policy.

5. Supervision & Team Collaboration

  • Provide direction and supervision to LPNs and CNAs as applicable
  • Participate in interdisciplinary team meetings and care coordination
  • Support clinical education and guidance for field staff

Qualifications

  • Graduate of an accredited School of Nursing (ASN or BSN required).
  • Current Florida Registered Nurse (RN) license in good standing
  • Minimum of 1 year nursing experience preferred
  • Home health, hospice, or acute care experience preferred
  • Current CPR certification required
  • Valid driver’s license, reliable transportation, and auto insurance
  • Ability to pass background screening and all compliance requirements
