Job description

Location: Citrus County, FL and surrounding areas (including Ocala/Marion County)
Employment Type: Full-Time
Schedule: Monday–Friday
Compensation: Base salary up to $100,000/year (DOE) + performance bonuses

About the Role

Bring high-quality primary care directly to patients where they live. In this community-based position, you’ll conduct home visits, manage chronic and acute conditions, and collaborate with a supportive care team to improve access and outcomes—while enjoying a consistent Monday–Friday schedule.

What You’ll Do
  • Conduct ~9 patient home visits per day across the assigned territory

  • Perform assessments, manage chronic/acute conditions, and prescribe per scope/DEA

  • Order and interpret labs/diagnostics; coordinate referrals as needed

  • Document encounters in eClinicalWorks (eCW) and close charts timely

  • Educate patients/caregivers on care plans, medications, and follow-up

  • Collaborate with physicians, care managers, and community resources

Requirements

  • Active Florida Nurse Practitioner (APRN) license

  • DEA certification

  • Valid driver’s license and reliable transportation (daily local travel)

Preferred

  • Experience in home-based primary care, community health, or house-calls

  • Strong communication skills and a patient-first mindset
