Job description

A Hospice Licensed Vocational Nurse (LVN) provides direct, compassionate bedside care and comfort measures to terminally ill patients, primarily focusing on pain management, symptom control, and emotional support for patients and families. Working under RN supervision, they implement the established plan of care, document patient changes, and assist with daily living tasks in homes or facilities.

Key Responsibilities

  • Direct Patient Care: Performs skilled nursing interventions, including wound care, medication administration, and monitoring vitals to manage pain and symptoms.
  • Documentation: Maintains accurate, timely clinical notes in electronic medical records regarding patient condition changes.
  • Family Education & Support: Educates family members on disease progression, symptom management, and medication administration.
  • Interdisciplinary Team Collaboration: Reports significant findings to the Registered Nurse (RN) or Case Manager and coordinates care with social workers and hospice aides.
  • Support Services: Provides emotional support to families, helping them manage anxiety and prepare for the end-of-life process.
  • Qualifications

  • Education: Graduate of an accredited vocational or practical nursing program.
  • Licensure: Active LVN/LPN license in the state of Texas.
  • Experience: Generally, at least one year of experience in hospice is preferred.
  • Skills: Strong communication, compassion, and the ability to work independently in home settings.
  • Requirements: Reliable transportation and CPR certification. 
  • Typical Working Conditions

  • Environment: Frequently involves visiting patients in their private homes, nursing homes, or inpatient hospice units.
  • Schedule: Often includes 8 or 12-hour shifts, with potential for on-call, weekends, and holiday rotations.
